What happened
while pulling concrete off of the chute to pour a Verti Block form the injured stepped down from the platform and placed his hand on the top of the form. One of the top latches on the form was unsecured and a co-worker reached up to close it. He did not see the injured 'd hand. The injured 's glove became caught in the gap pinching his pinky finger in the joint of the form until the co-worker released the latch.
