What happened

On [REDACTED] at [REDACTED] three maintenance associates were completing a repair on a loud squeaking conveyor. After the completion of the repair the team ran the equipment while collecting their tools to ensure the quality of the repair. [REDACTED] went down the ladder to get to the [REDACTED] so he can raise up to the bottom of the highway conveyor to retrieve his tools from [REDACTED]. [REDACTED] raised his right hand near an unguarded running chain waiting for his tools while [REDACTED] was trying to collect the tools. The space between the conveyors that the tools were going to go through was tight and not easy to navigate through. As [REDACTED] turned around he saw [REDACTED] going down on the [REDACTED] making mention that he cut his fingers while walking quickly to the maintenance shop. [REDACTED] immediately went down the ladder to catch up with [REDACTED] heard again that [REDACTED] cut his fingers obtained a phone from another associate and called [REDACTED] for an ambulanc
