EE alleges approx 1 day he had finished doing a delivery at the [REDACTED] store he was lifting the ramp but the ramp got stuck so he was using a tube to get it unstuck when the tube became loose causing him to loose his balance and he fell (approx 1-1.5 meters) onto his right arm specifically his right shoulder and his right little finger ring finger and right middle finger got bent.
