What happened
The associate was in the process of changing their propane tank on their PIT He connected the hose to the valve When the valve was opened he did not realize that the valve was cross threaded Propane gas shot out of the connection hitting the employee in the face This caused acute irritation to the right cheek and affected the vision in the right eye The employee reported that the proper PPE was on First aid was given and employee was taken urgent care for medical evaluation
