Before the incident

Field expert was assembling a racking structure with a partner on the north end of the building near the blending station To allow more room to work on the underside of the racking the field expert had used a forklift to lift the racking off the ground by a top side cross beam already put in place As the field expert was putting another cross beam in place on the underside of the racking another cross beam on the underside of the rack where the forklift was located suddenly dropped on the ground and the sound of the cross beam hitting the floor startled the employee and they stood up As the employee stood up the employee struck the lower thoracic vertebrae against the cross beam that was directly behind them
