What happened
Employee was blowing cellulose insulation into the open attic area of the a manufactured home in [REDACTED]. The employee climbed over the guardrails of the catwalksuspended walkway into the attic of the home. The insulation hose caught on the guardrail tugging on the end the employee was holding causing the employee to lose their footing and step onto the sheetrock ceiling and fell through to the floor. The employee fell approximately 8 ft and broke their leg on landing.
