Operator indicated that they had started seeking help for this condition approximately 2 months prior to speaking to the site OHN. Operator informed supervisor that during a follow-up appointment [REDACTED] prior to [REDACTED] that the surgeon wanted to schedule the first procedure for [REDACTED].
