Employee was picking a pallet when driving into aisle 12 he stopped to grab crates he would need for order. Employee grabbed four crates which he put on the empty part of his forks and the crates fell off of forks.

When employee went to put crates back into position he was standing close to his jack than he thought. The crate bounced off the guard of his jack into his forehead.

Trauma to forehead causing open wound.
