According to witness - employee was up on the scaffold maneuvering the head back into place when the employee slipped causing both the scaffolding board and employee to fall to the ground. The employee tried to catch himself on the way down but could not due to the grease on his gloves. It appeared that he hit the ground with his right knee first then his right shoulder followed by his right temple
