Before the incident [REDACTED] was working on getting the grabber bushings removed he had previous to the incident removed both the single and double grabbers. When hammering to remove the bearings a piece of the bearing flew into his cheek beneath the right eye causing a cut. [REDACTED] cleaned the cut immediately after and informed his supervisor over the phone of what had happened stating that he didn 't need medical attention. On the following [REDACTED] [REDACTED] came to me stating that he thought a piece still might be lodged in his upper cheek and that he would like to seek medical attention.

[REDACTED] was using a hammer to remove bearings when a broken piece of metal hit his cheek just below his right eye. While initially medical attention was refused the [REDACTED] returned to work and requested medical attention.
