Before the incident
IW was torch cutting. Another employee was cutting on another unprepared piece of steel the steel had a tire mounted on it about 20 yards from the IW.
What happened
As the other employee was cutting the unprepared steel it exploded and struck the IW 's left leg and hip. It knocked the IW to the ground.
