What happened
[REDACTED] was getting off a sit down fork truck and in the process hurt himself. He appeared that he had discomfort to his left leg around the hip thigh and knee area. He tried to stretch the area by walking around and sitting down but still had discomfort. I sent him to Concentra for evaluation and expecting their findings.
