Instructing employee on the the product code entry. Supervisor moved around the side of the forklift for a clear view of the rf screen
What happened
The employee after getting on to the equipment to enter the product code. The employee moved the control lever causing the forklift to move toward the supervisor as the lift was parked with the wheel turned. The supervisor was then pushed into the dock plate abruptly.
