Before the incident
filling a forklift cylinder in cage with propane. [REDACTED] had just filled a forklift cylinder with propane and unhooked from the filler valve and rubber gasket came outand sprayed propane onto his leg.
What happened
Driver was filling a forklift cylinder and when he unhooked from the filler valve the rubber gasket came out and valve started to sprayed propane driver turned to get away from the cylinder and the propane sprayed him on the back of his left leg.
