Loading handrails onto a trailer

On [REDACTED] while loading a 20ft long by 3 ft tall steel handrail onto a trailer using a backhoe the employee [REDACTED] was guiding the handrail when a chain securing it slipped [REDACTED] had noticed that the backhoe s thumb was not functioning properly and was informed but he continued the loading process Despite being warned that the chain was coming loose he proceeded as instructed During this process the handrail became unstable and struck [REDACTED] in the head Witnesses including [REDACTED] observed the incident and confirmed that the handrail not the chain was the object that caused the head impact The employee reported feeling fine immediately following the incident
