What happened
Employee used a forklift that was out of service to move a combo from one location to another location. The red forklift had a hydraulic hose leak noted on third shift. The employee did not fill out her preshift forklift inspection checklist. While using the red forklift hydraulic fluid sprayed into both of her eyes. Employee flushed both her eyes out in the eye wash station and waited on paramedics to arrive.
