Before the incident
Employee was moving pallets to the production line using the pallet jack.
What happened
While the employee was operating a pallet jack to move pallets the employee lost their footing and slipped while riding the pallet jack. This caused their foot to slip off the platform and land in front of the moving pallet jack. As the pallet jack continued to move the employee 's foot became trapped underneath it.
