Employee reports she bent over to pick up some food off the floor and struck her forehead on the computer desk next to the bed. Happened 2200. Employee kept working and reported it 2300. She was alert oriented x4. Employee applied ice pack to her forehead. Denied symptoms initally then c o nausea and localized pain shortly after midnight. Discussed her options for treatment. Initially declined to go to [REDACTED] then decided to go. I walked her to the [REDACTED] 0020. Seen and eval by [REDACTED] MD. Diagnosed with concussion. Given Tylenol by [REDACTED] nurse. Discharged and was picked up by her husband. Noted contusion with mild swelling left ant forehead.

Nausea Vomiting Head EE became nauseated after hitting her head on a computer desk.
