What happened

Accompanied her primary nurse and other supporting staff members to assist with de-escalation efforts with patient. Patient was continuing to escalate threatening all staff members present and slamming her door. Door was opened by this nurse in an attempt to request her to sit down on her bed and relax and to further advise that her behaviors would warrant a return to seclusion when she suddenly charged at me in a threatening manner and punched me in the top left side of my head while screaming . Patient was immediately intercepted by other staff and taken directly back to seclusion hospital police were called to the unit for assistance.
