At [REDACTED] as he was wiping down the tubes at the Soaking station in [REDACTED] the air suction tube came loose and fell on his forehead the distance of the impact is approximately 3 feet causing a small scratch on his forehead and it slightly bled. Partner came to the production office and he was first aid assistance rested for about 30 minutes and then he stated that he was fine to returned to work. Around [REDACTED] he returned into office stating that was feeling some pounding or dizziness in his head due to the previous incident AMR was called and he was taken to the nearby Hospital to check him out and ensure he does not have a contusion.
