Before the incident
Pallet jack has been tagged out and will be inspected by a technician. Rytec door was tested by [REDACTED] to replicate incident. After time under door door did descend down before hitting pallet jack and then automatically rising back up.
What happened
[REDACTED] pallet jack stopped and would not respond under Rytec door going from grocery dock to 55 degree dock. After an allotted time the door came down and struck [REDACTED] on the back of head and neck. At that point [REDACTED] 's jack responded and started moving again. Shortly after [REDACTED] complained of head and neck pain.
