Before the incident
Employee stated that he went down to the black liquor pond to inspect the pond pump dike The dike was full of black liquor He had a flashlight and saw that the liquor was running out of dike across the roadbed He changed out of his regular boots into his rubber boots in order to investigate so he walked to east edge of pump and went to call his boss He stepped in the liquor It was slippery and the next thing he knew that he was falling could not stop and just kept sliding forward The black liquor got all over his face and body
