Before the incident

At approximately [REDACTED] the [REDACTED] pump failed and the EoP Washer began to overflow The crew leader sent the field operator to investigate Upon entering the operating floor the field operator saw stock backing up in the EoP repulper and out of the washer hood doors The crew leader immediately cut the stock off and the field operator went downstairs to check on the [REDACTED] pump When the operator arrived they tried to get over to the pump but was unable to reach it due to the presence of high residual stock and gas The operator exited the bleach plant and evaluated tasks that still need to be completed to isolate the equipment and returned to the 4th floor to close valves to the EoP washer and repulper When they exited the bleach plant they began to exhibit signs of ClO2 exposure At no point did the teammate don an SCBA or recall hearing the area alarms
