Pulling a wrench to tighten a bolt.

What happened

At approximately [REDACTED] on [REDACTED] two Team Members were tightening bolts on crusher motor using a hammer wrench. The prior two bolts were able to be tightened with hammer wrench in the traditional manner. The bolt involved with incident did not leave room to use the hammer wrench with hammer due to minimal space. At this point the Team Members elected to use a pipe on the end of the hammer wrench to apply force to tighten up. In the action of applying force the bolt broke and TM lost balance falling backward from a squatting position and hit his back on a metal beam. Operations manager was contacted and instructed Team Lead to have TM contact WorkCare. WorkCare instructed TM to use OTC meds and ice on affected area of back. Both team members involved with task were also sent to do drug testing. Work care will follow up with TM in 24-48 hrs.
