What happened
per [REDACTED] and medical documentation: EE reports loading and unloading luggage onto aircraft and performing procedure to allow for airport departure reports L shoulder and upper back started hurting from the very beginning of work and throughout the day is worsened. By the end of the day pain was significant. MOI correlated by MD. The employee reported physical discomfort resulting from activities conducted outside of work and unrelated to job duties. No workplace conditions or incidents were identified as contributing factors. This note serves to document the employees disclosure and supports any necessary administrative considerations.
