On [REDACTED] the operator was placing approximately 24 5-gallon buckets of paint in a box. The buckets weight approximately 51 pounds. This was 4 'x4 ' so the buckets had to be lifted over the side of the box and the pallet approximately 54 from the floor. Another operator was assisting in doing this task. On [REDACTED] the operator made a passing comment about back pain to his supervisor and claimed he did not know how he injured his back but it could have been several things (sleeping in the incorrect position doing something at home or lifting paint buckets). On [REDACTED] the operator stated that he was sure it was from lifting the paint buckets on [REDACTED]. Amphibious was called on [REDACTED] and they conducted an e-visit and explained their treatment plan. On [REDACTED] the operator contacted the supervisor and stated they were going to another provider. The operator was released to work under light duty conditions and prescription medication.
