What happened
[REDACTED] was cleaning a filter on one of the regrind vacuum pumps. He had to remove the cover in order to do so. He placed it on top of the machine. The cover then fell and hit [REDACTED] in his forehead. During this work [REDACTED] was working with a contractor. [REDACTED] claims that he thought the contractor was holding the cover to prevent it from falling but he was not.
