What happened
[REDACTED] and [REDACTED] responded to a call at the [REDACTED] corrugated box caser because the head was stuck in upper position. While troubleshooting he decided the put his right hand through a gap on the plexiglass door to pry the head free. The head became loose started coming downward to place carton in the box and [REDACTED] hand was pinched between the caser wall and head of the machine.
