Before the incident
[REDACTED] was running the old palletizer went to the computer screen inputted some data. Noticed a stack was stuck at the gate about 10 yards up the line. When [REDACTED] went to get the stack he stepped on a slick spot on the cooler floor and lost his footing. Falling and unable to catch himself landed on his left shoulder. There were no witnesses.
