Before the incident
Drawing the patients blood.
What happened
Employee was drawing blood took out the needle applied a cotton ball and a band aid to the patients arm. I noticed that after taking the needle out that part of the butterfly was stuck inside of the clear suction chamber where the tubing goes to suction the blood. I tried to take it out by pulling and pushing on the rubber portion of the butterfly but it did not work.
