On [REDACTED] Line 4 switched to bulk pack. EE was assigned to the bulk pack area to do line attendant tasks. This consists of stacking pizzas shoving them into bags and placing them into boxes. EE primarily noted the motion of shoving pizzas into bags as causing gradually increased pain levels. By the 3rd day EE was having pain and reported it to her supervisor. Employee called off work on the [REDACTED] and was scheduled off on [REDACTED]. On [REDACTED] EE went to the ER because their pain had spread from their arm towards her upper back. ER gave her Flexeril and Tylenol also telling her to return to work on [REDACTED].
