Before the incident
I went into patient room with straight needle (green) tourniquet tape gauze alcohol pads and tubes. With the straight needle i stuck the patient saw some blood but not flowing took the straight needle out
What happened
When I took the straight needle out i unscrewed the vaccutainer and used the same vaccutainer for the butterfly drew the patients bloodwork and applied bandage and guaze when i started to collect all my trash and sharps to throw out is when i got stuck
