Unbeknown to me I was sitting in a faulty chair documenting my patient.

I began to document the in the patients chart I was assigned to when suddenly the back of the chair I was sitting in snapped abruptly and I fell sharply to the left side and felt a jolt in my lower back region. A screw fell from beneath.
