Before the incident
EE was machining swager die segments
What happened
EE was using a milling machine cutting swager die segments. As EE was completing his finish pass a Supervisor approached him and asked if he could help him find some bolts. EE finished the last cut and turned off the feed on the table. EE told the Supervisor where the bolts were located then EE removed the finish part and grabbed another die segment to start on another setup. EE was using 2 hands to set up a new segment the segment rolled into the vise and EE used his left hand to roll the segment back over in the vice when the EE pinky finger made contact with the quill of the mill that EE did not realize he left on and it was still spinning which caused a deep laceration to left hand pinky finger.
