Just before the incident the employee was straddling a 20-foot I-beam to cut it in half with a torch. The beam being cut was positioned at an angle across another I-beam with one section suspended in the air and not properly supported or secured.

After the suspended section was cut free it dropped onto the employee’s foot causing injury.
