Employee was doing his duties and attending to the moulder. He slipped and fell on combination cornmeal and flour. He immediately came to me and I asked him if he needed medical attention. He said no. I went and got an incident report and asked him to fill it out. He needed assistance filling out from his wife and brought it back the next day. [REDACTED] was there and saw the incident and filled out the proper paper work.
