Before the incident
Employee was walking in between the pan oiler on the bread line and brew tanks While doing so they stepped on a cardboard box that was covering the oil which slipped out from under them causing them to fall and injure their back There was a wet floor sign next to the cardboard the employee was wearing slip resistant boots and the employee reported the occurrence to their Supervisor right away The employee indicated that they did see the wet floor sign but thought the cardboard was covering water and would stick to the ground He was not aware that it was oil Their Supervisor asked them if they were ok and they confirmed yes they were and the Supervisor thought that they seemed fine the rest of their shift Later the employee called their Supervisor before it was time to come in and told them he would not make it in to work [REDACTED] that his back hurts and he cannot turn his head The Supervisor told him to make sure he goes to the doctor the [REDACTED]
