Before the incident
Employee was using a forklift to load in-process material totes onto a semi-truck.
What happened
While loading the semi-trailer the employee picked up 5 totes to place into the semi-truck trailer. While entering into the trailer the 5th tote struck the overhead door causing the tote to slide off of the stack onto the forklift overhead guard. The employee put their hand up through the guard to attempt to stop the tote.
