What happened
[REDACTED] tech had taken [REDACTED] to the bathroom and was trying to get PT back in bed. When [REDACTED] started trying to hit the [REDACTED] tech. [REDACTED] became very aggressive towards the tech. PT called for help. I came into the room to help get PT back in bed. PT got left arm free from the [REDACTED] tech and punch me in the left arm shoulder. PT continued to try and hit the staff. We were able to get PT back in bed. Doctor notified. Security called [REDACTED] placed in 4-point soft restraints per doctors ' orders. Nursing supervisor made aware of what happened. I have lump on my left arm from [REDACTED] punching me.
