Before the incident
[REDACTED] and [REDACTED] had just picked up a 200-pound pyramid anchor and needed another. [REDACTED] asked [REDACTED] to find a separate anchor that wasn 't attached by wire rope.
What happened
[REDACTED] placed his hand on the stack of anchors to steady himself as he walked around. As he did a loosely stacked pile of tied-together anchors slid down catching his lower leg and foot. Three 200-pound anchors fell causing the injury.
