Before the incident
IP was in the nonferrous processing area in the unprepared stainless bin standing between two heat exchangers stainless tube bundles using a power washer to clean dirt off of the bundle tubing.
What happened
While stepping back his foot got caught in the power washer hose causing him to stumble. In an effort to regain his balance he fell forward and hit the left side of his head on the heat exchanger resulting in laceration
