What happened

Patient was deemed able to transfer safely by PT and when time came to transfer to wheelchair when I went to stand her she was very weak and put all her weight on me and I injured my lower back. PT could not move her feet on her own. I tried sitting her back down onto her bed but she was not following directions due to her dementia. I felt pain immediately and during the transfer and at this time.
