At approximately [REDACTED] a forklift operator was in the process of storing newly received product in chamber 5D within [REDACTED]. Once he reached the entrance of the chamber the electric pallet jack and product did not clear the yellow bollards. The impact of the machine striking the bollards caused the operator to hit his head on the pallet jack guard resulting in a laceration above his left eye.
