Before the incident
Employee was moving some tires around so that the tread labels could be attached.
What happened
Employee was assigned to place tread labels on tires. The tires were double stacked on the dock and had to be down stacked in order to label. While in the process of moving the racks to an area where they could be down stacked the employee changed direction on the lift without allowing the load to stabilize causing tires to fall from the 2nd rack bouncing off the top of the Material handling equipment and sticking the employee in the back.
