[REDACTED] [REDACTED] I was assisting pt to bedside commode. pt stated they felt weak. Assisted pt to ground so they would not fall. After allowing the pt to rest for a minute helped pt stand and sit on the commode to urinate. Afterwards helped pt back to bed. Assessed pt and obtained vitals signs. This caused me to have to strain and lift pt in order to protect them I injured myself. abdomen is sore and I am worried about a possible hernia as this required a lot of effort to lift and move pt without injuring them.
