What happened

When applying the pts all necessary medical equipment to ambulate and moving his necessary medical equipment to get to the bathroom. The pt is not very steady on his feet and requires assistance when standing he ranges from a 1 person assist to a 2 person assist with a walker. I did not always have the help when moving him I may have strained my back by assisting him to move and walk. The pt had refused to use a bed side commode and urinal he insisted that he walk to the bathroom each and every time. He was getting up about once every 2 hours. We also had to boost him in bed several times using the boost mode on the bed it still was very difficult to move him properly. I had called out sick on my next shift [REDACTED] due to not being able to walk from the pain. I had him again on [REDACTED] and my back is hurting again from having this pt.
