Installing slide out
What happened
Team member was installing a slide out into the I-beam. The slide outs are 1 shorter than the axle spacing so the non-exit end of the slide missed the lower flange of the beam on the opposite side. As a result the non-exit side fell to the ground causing the exit end to slam upwards. Team members hand was caught in between the slide out arm and the top flange of the I-beam which resulted in his left little finger to be smashed.
