The employee was cleaning a ceiling tile with a wipe while standing on a 10 ft tall platform ladder.

The employee asked another employee to push the platform ladder a few feet forward while she was standing at the top instead of coming down and moving it safely. One of the ladder legs was pushed into a drain causing the ladder to tip over and fall with the employee falling as well.

The employee 's left ankle was broken in the fall.

Falling from the ladder as it tipped over and then hitting the concrete floor directly caused the break.
