Installing a tie bar nut

The tie bar nut ended up getting caulked causing it to not thread correctly resulting in the nut coming off the tie bar and slipping from his hands. On the way down it hit the ladder and dented the second step. When it hit the ladder the employee braced himself for impact causing him to feel discomfort in his left shoulder
