Before the incident
EE was in process of assembling the frozen block grinder. He and a co-worker were lining up the auger to be inserted into the grinder.
What happened
While attempting to push the auger into the hole he and a co-worker pushed the auger forward as opposed to pushing the auger holder forward thereby pushing the auger into the air with no support. It then fell upon the EE 's foot breaking it.
